Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Please take a moment to fill out this very important patient registration. If you cannot complete the forms online, please arrive 15 minutes early to complete the paperwork and allow time for us to enter your information.
We have reserved the appointment time specially for you. If you cancel your appointment with less than 48 hours notice, there will be a $25 fee. Our doctors time is valuable and a broken appointment is a loss to you and other patients who could have used the appointment time.
We look forward to your first visit! Please be advised that both doctors have allergies and are asthmatic. Please do not wear cologne or perfumes to your appointments. We apologize for any inconvenience this may render and appreciate your respect for the doctors health.
